Doctor-led functional medicine approach to sleep dysfunction addressing root causes — hormonal imbalance, gut disruption, cortisol dysregulation, nutrient deficiencies, and circadian misalignment — through targeted testing, supplementation, and MBSR-based sleep coaching.
Chronic sleep disruption affects an estimated 30–40% of Singapore's working population, and its consequences extend far beyond tiredness. Poor sleep accelerates biological ageing through telomere attrition and epigenetic drift, impairs glucose metabolism and immune function, elevates cortisol and inflammatory cytokines, and significantly increases the risk of cardiovascular disease, obesity, type 2 diabetes, and neurodegenerative conditions. Yet most clinical approaches to insomnia either prescribe sedative medication — which disrupts sleep architecture even as it increases sleep duration — or offer generic sleep hygiene advice without identifying the individual's specific root causes. NutraNourish's Sleep Restoration Programme applies functional medicine principles to identify and address the biological drivers of each patient's sleep dysfunction. The assessment begins with a comprehensive sleep history and the Munich Chronotype Questionnaire to characterise the patient's circadian phenotype (whether their sleep-wake preference is naturally early, intermediate, or delayed). This is combined with targeted biomarker testing selected from the clinic's functional panels. Hormonal drivers are assessed via DUTCH cortisol profiling (to identify HPA axis dysregulation — the most common biological cause of sleep-onset and sleep-maintenance insomnia), melatonin assessment, oestrogen and progesterone (particularly relevant in perimenopause, when nocturnal hot flushes and progesterone decline are major sleep disruptors), and thyroid function. Nutritional drivers assessed include magnesium, vitamin D, B6, tryptophan availability, and iron (ferritin insufficiency is strongly linked to restless legs syndrome). Gut health assessment may be incorporated where gut dysbiosis is suspected as a contributor — disturbed microbiome composition is associated with impaired production of serotonin (the melatonin precursor) and GABA. Inflammation markers identify systemic inflammatory load that can independently disrupt sleep architecture. The therapeutic programme combines targeted supplementation (magnesium glycinate, l-theanine, phosphatidylserine, 5-HTP, and melatonin at evidence-based doses and timings), circadian entrainment strategies (light exposure, meal timing, temperature protocols), and MBSR-based mindfulness practices for hyperarousal. Cognitive Behavioural Therapy for Insomnia (CBT-I) principles are woven throughout the coaching component. Hormonal and nutritional corrections are made on the basis of test results, and progress is tracked at follow-up appointments.
