The defining addition to this panel is creatine kinase (CK), also called creatine phosphokinase. CK is an enzyme found predominantly in muscle, heart, and brain tissue; it leaks into the bloodstream when muscle cell membranes are disrupted by intense mechanical load. Moderately elevated CK is normal immediately after heavy training, but persistently elevated CK outside expected post-exercise windows can indicate overtraining syndrome, rhabdomyolysis (severe muscle breakdown with kidney risk), or underlying myopathy.
