IFCOR's Men's Extra Prevention panel adds two high-value biomarkers to the Men's Basic panel: TSH and 25-OH vitamin D. While thyroid disorders are more common in women, hypothyroidism affects approximately 3–5% of adult men and often presents with non-specific symptoms — fatigue, weight gain, cold intolerance, or reduced libido — that are frequently attributed to lifestyle rather than investigated. Adding TSH ensures that thyroid function is included in the annual screen rather than requiring a separate visit. Vitamin D (as 25-hydroxyvitamin D) is relevant to men not only for bone health and immune defence but also because low levels are associated with reduced testosterone production. Several studies have found an inverse correlation between vitamin D deficiency and hypogonadism, and correcting deficiency through supplementation has been shown in randomised trials to raise testosterone in vitamin-D-deficient men. Given that the Czech Republic experiences low UV exposure for 5–6 months of the year, vitamin D deficiency is highly prevalent even in physically active men. The complete biomarker list: CBC with differential, Na/K/Cl, glucose, full lipid panel, ALT/AST/GGT/ALP, bilirubin, urea, creatinine, CRP, uric acid, 25-OH vitamin D, TSH, PSA, and urinalysis with sediment. Preparation and collection are identical to the Basic panel. PSA restrictions apply: no rectal examination or cycling for 24 hours, no biopsy for two weeks.
