Facial Fat Transfer (Lipofilling)

Facial volume restoration using the patient's own fat at FNKV's plastic surgery department. A small amount of fat is harvested by gentle liposuction, processed, and micro-injected into hollow areas — tear troughs, cheeks, temples, nasolabial folds or lips — replacing lost volume with living tissue rather than temporary filler. The department's microsurgical and reconstructive background (lipofilling is also used in its breast reconstruction programme) underpins the grafting technique. One area costs 20,000 Kč; multiple areas 36,000 Kč; planned top-ups 9,500 Kč.

Facial ageing is substantially a story of volume loss: fat compartments in the cheeks, temples and around the eyes deflate and descend, producing hollowness and shadows that skin-surface treatments cannot address. Autologous fat transfer (lipofilling) restores this volume with the patient's own living tissue. **How it works:** Fat is harvested with fine cannulas from the abdomen or thighs under local anaesthesia, purified, and injected in tiny aliquots across multiple tissue planes of the target area. Because each micro-graft must acquire its own blood supply, technique matters enormously — over-bolusing kills grafts, while careful micro-droplet placement maximises survival. Typically 50–70% of grafted volume establishes permanently; the published top-up session (9,500 Kč) exists precisely to titrate the final result after the take has declared itself at ~3 months. **FNKV's grafting pedigree:** Lipofilling is a core technique in the department's breast reconstruction programme (it appears on the clinic's reconstructive menu), so facial patients are treated by surgeons who graft fat at clinical volumes routinely — not as an occasional add-on. **What it treats:** Tear-trough hollows and lid–cheek junction, flattened cheeks and temples, nasolabial folds, marionette lines, thin lips (the clinic also lists lip augmentation with local tissue separately), and contour irregularities including post-traumatic or post-surgical defects. **Versus hyaluronic filler:** Fat is the patient's own tissue — no foreign material, no repeat purchases every 9–18 months, and a soft, natural integration. The trade-offs are a minor harvesting procedure, initial swelling of 1–2 weeks, and partial graft resorption requiring the possibility of a top-up. **Recovery:** Swelling and possible bruising at both donor and recipient sites for 1–2 weeks. The stable result is assessable at 3 months, when any top-up is planned.
