The LESC (Liposuction with External Simultaneous Cooling) cold-ultrasonic liposuction system addresses the primary limitations of conventional ultrasonic liposuction — tissue heating and thermal injury risk — by maintaining the probe tip at a controlled temperature of 25°C throughout the procedure, even as ultrasonic energy is delivered at 30–40 kHz. The cooled probe transmits ultrasonic cavitation energy into the fat layer while preventing any thermal build-up at the tissue interface, protecting nerves, vessels and the overlying skin from heat damage. The mechanism of fat removal combines ultrasonic cavitation with gentle tumescent suction. A tumescent solution (dilute local anaesthetic with adrenaline) is first infiltrated into the treatment zone; this solution serves triple duty — providing local anaesthesia, causing vasoconstriction to minimise bleeding, and creating a fluid environment within which the ultrasonic probe generates stable microbubbles. When these microbubbles collapse (acoustic cavitation), they release energy that disrupts adipocyte (fat cell) membranes, releasing triglyceride content into the extracellular fluid. The emulsified fat is then gently aspirated through the fine cannula attached to the 2 mm probe. The entire process is mechanically precise: the small probe diameter allows treatment of contour-sensitive areas (submental, inner arms, knees) that larger cannulas cannot access safely. A notable secondary effect of the LESC method is skin tightening. The ultrasonic energy and the physical suction at the interface between the fat layer and the overlying dermis create a mechanical stimulation of dermal fibroblasts, promoting collagen contraction and new collagen synthesis in the reticular dermis. Clinically this means the skin over the treated area does not typically look loose after fat removal — in contrast to aggressive conventional liposuction where skin retraction may be a concern.
