Fractional photothermolysis with an ablative CO2 laser represents the most powerful resurfacing modality available in outpatient aesthetic medicine. The Pixel CO2 system at Artmedica uses a holographic diffractive lens mounted on the handpiece to split the continuous CO2 beam into a grid of 49 or 81 precisely spaced microbeams. Each microbeam creates a microscopic ablation column — called a micro-treatment zone (MTZ) — that extends from the surface of the epidermis down through the papillary dermis and into the upper reticular dermis. Within each MTZ the tissue is vaporised cleanly with the CO2 wavelength's high water absorption; surrounding the ablation core is a zone of controlled coagulation that provides haemostasis and drives the thermal collagen denaturation that initiates remodelling. Because the MTZs cover only 20–30% of the total surface area treated in one pass, the intervening 70–80% of tissue — containing viable keratinocytes, pilosebaceous units and dermal fibroblasts — remains physically undisturbed. These undisturbed zones act as biological reservoirs: keratinocytes migrate laterally across each MTZ within 24–48 hours to restore epidermal continuity, drastically reducing healing time compared to fully ablative CO2 treatment. Meanwhile the dermal response to the thermal injury pattern — a cascade of inflammatory cytokines, TGF-β release and fibroblast activation — drives new collagen synthesis over two to three months, producing long-lasting structural skin improvement. At Artmedica Clinic Prague topical anaesthetic cream (EMLA) is applied 40 minutes before the session. During treatment patients experience warmth and a stinging sensation as each pulse is delivered; the 20-minute session covers the face efficiently. Immediately afterwards the skin is red, swollen and may feel rough. Days one and two bring peak swelling and redness; by days three to six the ablated microcolumns begin to surface and shed as a fine powdery exfoliation. Make-up can be applied after several days once re-epithelialisation is complete. The skin appears pink and new beneath; this pink fades over four to eight weeks. The remodelling response matures fully at three months. Strict SPF 50+ use is mandatory for three months after treatment — the new skin has minimal UV protection and pigmentation risk is elevated. The result from a single intensive session typically lasts three to five years as the new collagen architecture resists further ageing. For significant acne scarring or deeper photoaging a second session may be performed at six to twelve months.
