Affidea Praha's cardiac CT service combines the two principal non-invasive coronary imaging techniques — coronary artery calcium scoring (CAC score) and CT coronary angiography (CTCA) — in a comprehensive appointment that provides a complete picture of coronary artery health. The examination begins with the non-contrast ECG-gated acquisition for calcium scoring, producing the Agatston score that quantifies calcified plaque burden. This is followed by a contrast-enhanced CTCA sequence that visualises both calcified and non-calcified (soft) plaque, evaluates the anatomy and patency of all major coronary branches, and assesses the cardiac chambers and pericardium. The GE Revolution GSI scanner's spectral imaging capability is particularly advantageous for cardiac work: low-dose protocols reduce radiation compared to conventional CT, fast detector readout minimises artefact from cardiac motion, and dual-energy reconstructions can better characterise plaque composition and reduce blooming from calcified lesions that might otherwise obscure adjacent lumen. The combined cardiac CT is indicated for patients with intermediate pre-test probability of coronary artery disease, chest pain syndromes where stress testing is inconclusive or not feasible, and for comprehensive risk stratification in individuals with multiple cardiovascular risk factors. It is also appropriate as a pre-operative coronary evaluation before non-cardiac surgery in selected patients. The radiology report covers the Agatston score with percentile ranking, a segment-by-segment coronary artery assessment, characterisation of any stenoses as non-obstructive or obstructive, and cardiac structure evaluation (left ventricular function estimate, pericardium, aortic root). Findings that indicate significant obstructive disease are flagged for urgent cardiologist referral.
