The FOTONA 4D is a signature multi-pass laser rejuvenation protocol delivering four distinct treatment modes in a single session using Fotona's dual-wavelength Er:YAG and Nd:YAG laser system. Each pass addresses a different depth and mechanism — intraoral tightening, deep coagulation for tone, bulk heating for collagen remodelling and a superficial smoothing pass — collectively targeting wrinkles, skin laxity, enlarged pores and uneven texture without surgery or extended downtime. AE Derm is equipped with the Fotona system, one of the most clinically validated laser platforms in aesthetic medicine.
The FOTONA 4D protocol was developed by Fotona, a Slovenian medical laser manufacturer with one of the longest track records in clinical laser applications. The system houses two laser wavelengths in a single device: Er:YAG at 2940 nm, which is highly absorbed by water and acts superficially with precise ablative or non-ablative effects depending on pulse duration, and Nd:YAG at 1064 nm, which penetrates more deeply into the dermis and subcutaneous tissue for coagulation and bulk heating without surface ablation. The 4D protocol applies these wavelengths across four sequential passes, each named after its specific mode. SmoothLiftin uses the Nd:YAG in a non-ablative intraoral mode: the handpiece is briefly placed inside the mouth to heat and tighten the mucosa and submucosal tissue, delivering a tightening effect to the perioral skin and nasolabial area from inside. FRAC3 uses the Nd:YAG in a fractional, non-ablative mode targeting superficial pigmentation and diffuse redness. PIANO uses the Nd:YAG in a rapid scanning mode for deep bulk heating of the dermis and SMAS (superficial muscular aponeurotic system), stimulating collagen remodelling and producing a lifting and firming response. The SupErficial Er:YAG pass completes the treatment with a very superficial, light peel that smooths the skin surface, refines pores and improves texture. At AE Derm MUDr. Helena Závodská performs each pass according to the patient's skin type, laxity and specific concerns. The treatment is well tolerated with topical anaesthetic cream; patients typically experience warmth and mild redness during and immediately after the session. Post-treatment redness resolves within hours to a day or two. The FOTONA 4D is generally performed as a course of three to four sessions spaced three to six weeks apart for cumulative collagen stimulation, with annual maintenance sessions thereafter. It addresses forehead, cheeks, jaw, neck and perioral zones and can be combined with other laser modes available on the Fotona system for more targeted concerns.
