Acne vulgaris and rosacea are both inflammatory skin conditions but with distinct pathophysiology, requiring different emphases in treatment. Acne involves hyperkeratinisation of the hair follicle, colonisation by Cutibacterium acnes (formerly P. acnes), excess sebum production and an inflammatory cascade generating papules, pustules and, in severe cases, nodules and cysts. Rosacea is characterised by chronic facial redness, visible telangiectasias (broken capillaries), flushing and sometimes papulopustular lesions, driven by neurovascular dysregulation and an abnormal innate immune response. At AE Derm MUDr. Helena Závodská builds an individual treatment protocol for each patient after assessing the type and severity of acne or rosacea, skin sensitivity and the presence of active inflammation. For acne, the FOTONA 4D laser can be applied in dedicated acne modes: the Nd:YAG wavelength heats and disrupts the sebaceous gland, reducing sebum output and creating an inhospitable environment for bacteria; it also coagulates the inflamed vessels surrounding active lesions. LED blue-light phototherapy complements laser by producing reactive oxygen species within C. acnes cells, destroying the bacteria through photodynamic action even in non-laser sessions. Chemical peeling — particularly salicylic acid or low-concentration TCA for acne-prone skin — resolves the keratinised plug that blocks the follicle and brings fresh skin surface cells into contact with topical treatments. For rosacea, the treatment focus shifts to vascular management. The Nd:YAG 1064 nm wavelength at specific fluences selectively heats haemoglobin in the telangiectasias, causing coagulation of the dilated capillaries without damaging the surrounding skin. LED red-light and near-infrared sessions reduce baseline inflammation and skin reactivity. Chemical peels for rosacea are selected cautiously — very superficial lactic or mandelic acid formulations that do not provoke flushing — and are used for maintenance rather than deep resurfacing. Most acne and rosacea patients benefit from combining modalities across multiple sessions rather than relying on a single approach. MUDr. Závodská typically schedules laser, phototherapy and peel sessions alternately to allow skin recovery between more intensive interventions while maintaining continuous momentum in the treatment programme.
