May include cardiopulmonary exercise testing (CPET), chest CT, colonoscopy, advanced prostate screening, DEXA bone density scan, and coronary CT angiography based on individual risk factors.

The Bespoke Executive Screen takes the already comprehensive Core programme and adds clinically directed investigations tailored to each patient's risk profile, family history, and health concerns. Rather than a fixed menu, the additional components are selected through a pre-assessment consultation where the clinical team reviews the patient's medical background and designs an assessment day that addresses their specific priorities. Potential additions include cardiopulmonary exercise testing (CPET), which measures peak oxygen consumption, anaerobic threshold, and cardiac response under controlled exertion — the gold standard for assessing cardiovascular fitness and identifying exercise-limiting pathology. Chest CT provides detailed imaging of the lungs and mediastinal structures beyond what standard lung function tests reveal. Coronary CT angiography visualises the coronary arteries directly, identifying calcified and non-calcified plaque that may indicate significant atherosclerotic disease before it causes symptoms. For men with elevated PSA or a family history of prostate cancer, enhanced prostate screening protocols are available. DEXA scanning provides precise bone mineral density measurements and body composition data that complement the MRI body composition analysis in the core screen. Colonoscopy may be recommended for patients over 50 or those with relevant family history or symptoms. The bespoke approach means that the assessment day may extend beyond the standard core screen schedule depending on the scope of additional investigations selected.
