Lower eyelid ageing manifests differently from upper eyelid ageing, which is why the surgical approaches differ substantially. At V CLINIC Brno, lower blepharoplasty addresses two distinct presentations: the patient with excess herniated fat creating visible bags under the eyes, and the patient with under-eye hollowing and a tear trough deformity where the problem is volume loss rather than excess. For patients with fat-bag puffiness, the procedure uses an incision just below the lower lash line to access and remove the herniated fat compartments. For patients with hollowing — or a combination of both — fat transposition is performed, repositioning the fat inferiorly into the tear trough to restore volume without requiring external filler. Canthopexy (repositioning of the outer corner of the lower eyelid) is incorporated whenever the lower lid position requires stabilisation to prevent post-operative lower-lid laxity. Surgery takes 30–45 minutes under local anaesthesia; patients are discharged the same day. Internal absorbable sutures and fine external sutures are used, with external suture removal at day 7. Bruising and mild swelling are typical for one to two weeks, with the final result visible at approximately two months. Lower blepharoplasty is frequently performed in combination with upper eyelid surgery (four-lid blepharoplasty), mid facelift, or deep plane facelift to address the periorbital and mid-face zones together, maximising the cohesion of the overall rejuvenation.
