The incision is placed just below the lower lash line; for hollow areas, fat is transposed rather than removed to restore volume. The 30–45 minute procedure uses local anaesthesia, with suture removal at day 7 and full results visible at approximately two months. It is frequently combined with upper eyelid surgery or mid facelift for comprehensive periorbital rejuvenation.
About this treatment
Lower eyelid ageing manifests differently from upper eyelid ageing, which is why the surgical approaches differ substantially. At V CLINIC Brno, lower blepharoplasty addresses two distinct presentations: the patient with excess herniated fat creating visible bags under the eyes, and the patient with under-eye hollowing and a tear trough deformity where the problem is volume loss rather than excess.
For patients with fat-bag puffiness, the procedure uses an incision just below the lower lash line to access and remove the herniated fat compartments. For patients with hollowing — or a combination of both — fat transposition is performed, repositioning the fat inferiorly into the tear trough to restore volume without requiring external filler.
