Blepharoplasty is one of the most precisely technical procedures in facial plastic surgery — the eyelid is a thin, mobile, anatomically complex structure and any asymmetry in skin excision, fat management, or wound closure is immediately visible — and Dr. Alavi's combined training in Head & Neck surgery and facial plastic surgery provides the anatomical expertise required for consistent, natural outcomes. Upper blepharoplasty addresses dermatochalasis — the accumulation of excess skin on the upper eyelid that creates a heavy, hooded appearance — and in more advanced cases can impair the superior visual field. Through an incision precisely placed in the natural upper lid crease, excess skin is excised, pre-aponeurotic fat is conservatively removed or repositioned where prominent, and the wound is closed with fine sutures to produce a refined, open appearance to the upper eyelid with a minimal scar concealed within the crease. Lower blepharoplasty addresses under-eye bags — the prolapse of orbital fat through a weakened orbital septum that creates persistent puffiness below the eyes — and excess lower eyelid skin. The transconjunctival approach (incision inside the lower lid) removes or redistributes fat without a skin incision and is suited to younger patients without significant lower lid skin laxity; the transcutaneous approach (external incision below the lashes) allows concurrent skin excision in patients with lower lid skin excess. Dr. Alavi also performs concurrent eyebrow elevation and forehead skin tightening in conjunction with blepharoplasty to address brow ptosis that compounds upper eyelid hooding — a common finding in patients who present for upper eyelid surgery who also have descended brow position.
