This three-enzyme panel provides the essential assessment of liver function used in routine clinical practice worldwide. ALT (alanine aminotransferase) is the most liver-specific of the three — it is found predominantly in hepatocytes and is released into the bloodstream when liver cells are damaged. Mild ALT elevation (1-3x upper normal) is the most common biochemical abnormality in the general population, most frequently caused by non-alcoholic fatty liver disease (NAFLD), which affects an estimated 25% of European adults. AST (aspartate aminotransferase) is found in liver, cardiac muscle, skeletal muscle, kidneys, and brain. While less liver-specific than ALT, the AST:ALT ratio provides diagnostic information — a ratio above 2 strongly suggests alcohol-related liver disease, while a ratio below 1 is typical of NAFLD. In advanced fibrosis and cirrhosis, the ratio tends to rise above 1 regardless of aetiology. GGT (gamma-glutamyl transferase) is the most sensitive indicator of biliary disease and is particularly responsive to alcohol intake — even moderate alcohol consumption causes GGT elevation in many individuals. Isolated GGT elevation with normal ALT and AST is one of the most common incidental findings in routine blood work and frequently reflects alcohol use or medication effects rather than significant liver disease. Together, these three enzymes detect the vast majority of liver pathology that presents in ambulatory care. For patients on medications known to affect the liver (statins, methotrexate, antifungals, antiretrovirals), regular monitoring with this panel is standard practice. Albumin g/dL
