This three-enzyme panel provides the core liver function assessment used globally in clinical practice. ALT (alanine aminotransferase) is the most hepatocyte-specific of the three markers — it is predominantly found in liver cells and released into the bloodstream upon cellular injury. AST (aspartate aminotransferase) is distributed across liver, cardiac muscle, skeletal muscle, kidneys, and brain. While less liver-specific than ALT, the AST-to-ALT ratio carries significant diagnostic value: a ratio exceeding 2 strongly points to alcohol-related liver disease, whereas a ratio below 1 is typical of NAFLD. In advanced fibrosis and cirrhosis, the ratio tends to rise above 1 irrespective of the underlying cause.
GGT (gamma-glutamyl transferase) is the most sensitive indicator of biliary disease and responds prominently to alcohol intake — even moderate consumption elevates GGT in many individuals. Isolated GGT elevation with normal ALT and AST is one of the most common incidental laboratory findings, frequently reflecting alcohol use or medication effects (statins, antiepileptics, paracetamol) rather than serious hepatic pathology.
