Dr Ng performs open rhinoplasty via a transcolumellar incision, which provides full visualisation of the nasal cartilage framework and enables precise, reproducible modifications. The approach is tailored to Oriental nasal anatomy: the goal is to enhance the bridge with a silicone or Gore-Tex implant, refine the nasal tip using cartilage grafts harvested from the ear, septum, or rib (depending on the degree of augmentation required), and address the nasal vault and alar base where appropriate. Dr Ng's philosophy prioritises adequate tip projection and natural cartilage support over aggressive alar reduction, avoiding the scarring and nostril distortion that can result from overly zealous base narrowing. When rib cartilage is required — typically for revision cases or patients needing significant structural augmentation — general anaesthesia is used for patient comfort and surgical precision. Standard cases are performed under sedation or general anaesthesia as day surgery, with a duration of 4–6 hours for complex cases. A nasal splint is applied post-operatively and removed at one week; swelling resolves progressively over weeks to months, with final results visible at 6–12 months.
