The procedure begins with a small blood draw — typically 10 to 30 mL — which is then centrifuged in a two-stage process to separate the red blood cell fraction from the buffy coat layer containing the concentrated platelets. The resulting PRP has a platelet concentration several times higher than that of whole blood, along with a concentrated suspension of growth factors naturally secreted by platelets upon activation: platelet-derived growth factor (PDGF), transforming growth factor-beta (TGF-β), vascular endothelial growth factor (VEGF), insulin-like growth factor (IGF), and epidermal growth factor (EGF), among others. When injected intradermally or placed on the periosteum of the tear trough, these growth factors activate local fibroblasts, stimulate new collagen and elastin synthesis, improve local microcirculation, and promote tissue regeneration. In the periorbital context, this neocollagenesis and improved vascular support can reduce the visibility of the infraorbital dark circles that arise from thin skin, inadequate fat, and showing-through of underlying vasculature. The treatment also addresses the early skin laxity and fine crepe-like lines of the lower eyelid skin that develop with age as dermal collagen and elastin decline. Unlike hyaluronic acid under-eye fillers, PRP carries no risk of the Tyndall effect (the blue-grey discoloration from shallow HA gel placement near the surface) and uses autologous material, eliminating hypersensitivity or foreign body concerns.
