The Fraxel Dual at Skinly Aesthetics is the industry-standard fractional non-ablative laser device manufactured by Solta Medical, offering two complementary wavelengths in a single platform that address different tissue targets and treatment depths. The 1927nm thulium wavelength has high water absorption in the superficial skin layers, depositing the majority of its energy in the epidermis and superficial papillary dermis — making it the primary choice for targeting epidermal pigmentation, melasma, sunspots, solar lentigines, actinic keratoses, and overall skin tone evenness. The 1550nm erbium-doped fiber wavelength penetrates into the mid-to-deep reticular dermis, where its thermal coagulation columns stimulate fibroblast activity, elastin production, and collagen neogenesis — the primary remodelling mechanism for reducing acne scars, surgical scars, wrinkles, and skin laxity. The critical principle of both wavelengths is the fractional delivery pattern: rather than treating the entire skin surface, the Fraxel creates thousands of microscopic columns of thermal coagulation — called MTZs (Microscopic Treatment Zones) — separated by intervals of completely untreated tissue. The intact tissue bridges contain viable keratinocytes and fibroblasts that rapidly repopulate the treated columns, dramatically accelerating healing compared to fully ablative laser resurfacing. The density of MTZs (the percentage of skin surface treated per pass) is adjustable, allowing Dr. Schwarzburg to calibrate the aggressiveness of treatment based on the indication and the patient's tolerance for downtime. Mild redness, swelling, and a sandpaper-like skin texture (as treated microcolumns shed) characterise the one to three days of recovery. A course of three to five sessions spaced four to six weeks apart is associated with cumulative improvement in all indications, with each session driving a new cycle of remodelling.
