Fractional Laser Resurfacing (Youlaser MT)

Fractional laser resurfacing at Skinderm Ostrava uses the Youlaser MT dual-wavelength system to create thousands of microscopic ablative columns in the skin — triggering intense localised healing that resurfaces texture, reduces scar depth, and corrects uneven pigmentation. Unlike traditional ablative lasers that remove the entire surface, fractional delivery leaves healthy skin between each column intact, dramatically accelerating recovery. The procedure takes 30–45 minutes depending on the area treated, with a recovery period of 3–7 days of skin peeling.

Fractional laser resurfacing with the Youlaser MT works in the fractional ablative mode, using the 10,600 nm CO2 wavelength (and optionally the 1540 nm erbium glass for a combined effect) to create a precise grid of microscopic ablative zones — called micro-thermal treatment zones (MTZs) — in the skin. Each MTZ removes a narrow column of tissue from epidermis through dermis, creating a controlled wound. The surrounding un-treated tissue provides a reservoir of healthy cells that rapidly migrate in to close each column, meaning the thousands of tiny wounds heal in parallel far faster than a full-surface ablation would. The thermal damage from the ablative columns also extends laterally into the surrounding dermis, denaturing existing collagen and stimulating fibroblast recruitment. This dual response — re-epithelialisation from above and neocollagenesis from within — produces both a surface refinement (smoother texture, reduced irregular pigmentation) and a deeper structural improvement (tighter, firmer dermis with better attenuation of scar contours). At Skinderm, the physician selects the treatment density (percentage of surface covered by MTZs) and the depth based on the indication. Higher density and depth settings produce more dramatic results but also extend recovery. Before the procedure, topical anaesthetic cream is applied for 30–45 minutes. During treatment, patients feel a burning or prickling sensation; the Youlaser MT's cooled tip helps manage surface temperature and comfort. After the session, the skin is red and swollen for 1–2 days, then begins peeling from day 3–5 as the treated columns extrude. This peeling phase lasts 3–7 days depending on treatment intensity; keeping the skin moisturised during this phase is important. When the peeling resolves, the new epidermis is revealed — initially pink, fading over 2–4 weeks to the patient's normal tone. SPF 50 is mandatory throughout recovery. Best results for acne scarring or significant textural concerns require 2–4 sessions spaced 6–8 weeks apart. Fine wrinkles and mild pigmentation may respond in 1–2 sessions. The clinic also offers spot treatment of small areas (individual scars, single zone) at reduced cost. Contraindications: pregnancy, nursing, active acne in the treatment area, keloid tendency, isotretinoin within 6 months, photosensitising medications, very dark skin types without patch test (Fitzpatrick V–VI).
