The system uses Ultrasound Imaging guidance — a real-time visualisation layer built into the handpiece — to allow the treating provider to see the tissue layers being targeted before and during energy delivery, ensuring that thermal coagulation points (TCPs) are placed accurately at the intended tissue depths (1.5 mm, 3.0 mm, and 4.5 mm) without energy being deposited in unintended planes such as nerves or blood vessels. The 4.5 mm depth transducer targets the superficial muscular aponeurotic system (SMAS) — the same fibromuscular layer tightened in surgical facelifts — allowing Ultherapy to achieve a degree of structural lifting that surface-applied devices that only reach the dermis cannot replicate. At each focal point, the converging ultrasound waves create a precise TCP where temperature briefly exceeds the coagulation threshold for protein, triggering an acute heat-shock response and subsequent collagen remodelling and contraction at that micro-focus point. The aggregate effect of hundreds of TCPs distributed across the treatment zone creates a gradual overall lifting and tightening force as the new collagen matures over two to three months post-treatment. Clinically, patients notice brow lifting (reducing upper-lid heaviness), submental and neck skin tightening for early jowling and loose neck skin, and general skin density improvement on the lower face.
