The open technique places a small incision across the columella (the tissue between the nostrils), lifting the skin to expose the full cartilage framework. This gives the surgeon maximum visibility for precise cartilage modelling, correction of the tip, bridge reduction or augmentation, and asymmetry correction. Scarring across the columella fades to near-invisibility within six to twelve months in most patients.
The closed technique works entirely inside the nostril with no external incisions, avoiding any visible scarring and typically offering a slightly faster initial healing phase. It is selected when the degree of correction is more limited or when the patient's anatomy is favourable.
