The test requires the subject to rest quietly while breathing through a sealed canopy or mouthpiece connected to a metabolic cart that measures expired oxygen and carbon dioxide concentrations. From the ratio of O₂ consumed to CO₂ produced (the respiratory quotient) and the total volumes exchanged, the system calculates ATP production and thus caloric expenditure per unit time. The result is a direct measurement of how many calories the individual's body burns at rest to maintain vital functions — breathing, circulation, thermoregulation, organ function, and cellular maintenance — accounting for all individual variation in lean mass, thyroid function, hormonal status, and genetic metabolic rate that standard predictive equations (Harris-Benedict, Mifflin-St Jeor) cannot capture.
