Under-eye bags form when the fat that cushions the eyeball pushes forward against a weakened lower-lid septum, often combined with skin laxity and a hollow tear-trough groove beneath. Creams and injectables can only camouflage this anatomy; lower blepharoplasty corrects it. Depending on findings, the surgeon removes or — increasingly — repositions the protruding fat pads to fill the tear trough, and excises a conservative strip of excess skin through an incision placed millimetres below the lash line, where it heals to near-invisibility. In younger patients with bags but no loose skin, the fat can be addressed from inside the eyelid (transconjunctival approach), leaving no external scar at all.
