The surgeon plans the approach based on the extent of ptosis and the volume increase desired. Mild ptosis may be addressed through a periareolar incision that allows both implant placement and a crescent-shaped tissue tightening. More significant descent typically requires a vertical or inverted-T pattern, similar to a full mastopexy, performed concurrently with implant insertion. Surgical time is 2–2.5 hours.
Recovery follows broadly the same course as standard augmentation: supportive bra immediately post-operatively, restricted upper-body activity for four to six weeks, and implant settling over three to six months. The additional skin-excision component adds slightly more bruising and swelling in the first two weeks. Scar maturation takes 12–18 months; the incision lines fade considerably but remain permanently visible.
