Scar revision surgery exploits the plastic surgeon's expertise in wound geometry, tissue tension, and closure technique to replace an existing suboptimal scar with a finer, better-oriented one. Not all scars are amenable to surgical revision — some scars in high-tension areas or with ongoing collagen overactivity require combination treatment — but the majority of widened, raised, or irregular scars from trauma or previous surgery can be meaningfully improved. MUDr. Kloud's approach to scar revision begins with assessment at consultation: the scar's length, width, orientation relative to skin tension lines, texture, colour, and any functional component (tethering to underlying tissue, movement restriction) are evaluated. This determines whether surgical excision alone will be sufficient, or whether adjuncts such as Z-plasty (geometric scar rearrangement to redirect tension), W-plasty (irregular incision to break up the linear visibility), or tissue rearrangement flaps will produce a superior outcome. The standard technique involves excision of the existing scar with a thin elliptical design, followed by meticulous wound closure in layers — deep absorbable sutures to take tension off the skin surface, and fine surface sutures or tissue glue to align the skin edges with minimal trauma. The result is a narrower scar whose wound edges are in precise apposition — the primary determinant of final scar width. Pricing is length-based: CZK 3,000 for scars up to 5 cm, CZK 5,000 for 5–10 cm, and CZK 7,000 for longer scars. Complex scars involving multiple geometric corrections, keloid-prone areas, or functional restrictions may warrant a consultation to discuss the most appropriate combined strategy. Post-operative care after scar revision is as important as the surgery itself. Silicone gel or sheeting is applied to the healing scar from day 14 and continued for three to six months. Sun protection of the scar with high-SPF sunscreen is mandatory for the entire maturation period (up to twelve months) to prevent post-inflammatory hyperpigmentation. Gentle scar massage is introduced once sutures are removed and the surface is intact. The final result of a surgical scar revision is assessed at six to twelve months, when collagen remodelling is complete.
