The procedure begins with detailed mapping of lift vectors based on facial anatomy assessment. Local anaesthetic is infiltrated into the subcutaneous plane along each planned thread tract; once anaesthesia is adequate, a thin introducer cannula is passed through a small entry point and positioned at the target depth. The thread is deployed inside the cannula so that the barbs or cones open and engage surrounding tissue; the cannula is withdrawn while the thread remains anchored. When the free end is pulled, tissue advances along the barbs, repositioning the ptotic soft-tissue component. Multiple threads are placed in a fan or mesh configuration depending on the area — typically four to eight threads per side for a standard midface lift. The thread is trimmed flush with the skin at the entry point, which is closed with a small adhesive strip.
