4D Lifting — Fotona Multi-Mode Laser

4D Lifting at Petra Clinic uses the Fotona Dynamis laser system in a four-mode protocol that addresses skin laxity, texture and oral volume simultaneously — including an intraoral mode where the laser handpiece is applied inside the mouth to tighten the mucous membranes of the inner cheeks, palate and lip edge. The sequential delivery of four laser modes (SmoothLiftin, FRAC3, PIANO and SupErficial) stimulates collagen at four distinct tissue depths without ablating the skin surface, meaning no anaesthetic is required and most clients return to activities with approximately two days of mild post-procedure precautions. The result is a rounded, volumised facial contour that cannot be achieved by external surface treatments alone.

The 4D Lifting protocol at Petra Clinic employs the Fotona Dynamis Nd:YAG and Er:YAG laser platform in a four-step sequence designed to address facial ageing at multiple anatomical levels. The first step, SmoothLiftin, uses a non-ablative intraoral handpiece applied to the inner lining of the mouth — buccal mucosa, soft palate and lip mucosa — delivering bulk heating to the submucosal connective tissue, which contracts and creates visible volumisation of the lower face and lips from the inside out. Step two, FRAC3, delivers a non-ablative fractional Nd:YAG pulse to the outer facial skin, targeting deeper chromophore deposits and heterogeneous tissue imperfections including pigmentation and vessels. Step three, PIANO, delivers Er:YAG energy in a rapid-scanning ultra-long pulse to create bulk sub-surface heating — essentially an RF-like deep thermal effect using laser energy — that drives significant collagen remodelling. The fourth step, SupErficial, applies a light ablative Er:YAG polish to the skin surface, removing superficial dead cells and improving texture and tone without creating a wound.
