HbA1c measures the percentage of haemoglobin molecules that have been glycated (bound to glucose) over the 8–12 week lifespan of red blood cells. This provides a time-averaged picture of glycaemic control that is unaffected by day-to-day variation, fasting status, or acute stress responses. The diagnostic thresholds are well established: HbA1c below 5.7% is normal; 5.7–6.4% indicates pre-diabetes; 6.5% or above meets the diagnostic criterion for type 2 diabetes. Pre-diabetes is the stage where lifestyle intervention is most effective — the landmark Diabetes Prevention Program showed that moderate exercise and 5–7% weight loss reduced diabetes progression by 58%. Pre-diabetes affects an estimated 7–8 million people in Germany, and the majority are unaware. At €11.66 plus blood draw (under €16 total), annual HbA1c screening is a practical and cost-effective habit for anyone with risk factors: age over 45, overweight, family history, gestational diabetes, or sedentary lifestyle. HbA1c has limitations: it can be falsely low in conditions that shorten red blood cell lifespan (haemolytic anaemia, significant bleeding, chronic kidney disease on dialysis) and falsely elevated in iron deficiency. For these populations, fasting glucose or oral glucose tolerance testing may be preferable. For diabetes management monitoring, HbA1c every 3–6 months tracks treatment effectiveness. Target is typically below 7.0% for most diabetic patients, with individualised targets for elderly or complex patients.
