Rhinoplasty (Closed / Scarless Technique)

Dr. Gary Motykie's signature closed (scarless) rhinoplasty uses an entirely intranasal approach — no external incision on the columella — to reshape the nose for cosmetic or reconstructive goals. Approximately half of Dr. Motykie's rhinoplasty caseload comprises complex revisions referred from other surgeons. Open rhinoplasty, ethnic rhinoplasty, and post-traumatic nasal reconstruction are also performed. Computer imaging is used during consultation for result visualisation. The defining feature of Dr. Motykie's rhinoplasty approach is his commitment to the closed (scarless) technique as the default approach: all surgical incisions are placed entirely inside the nostrils, eliminating the small external scar across the columella (the skin bridge between the nostrils) that characterises the open rhinoplasty approach. In the closed technique, the surgeon works through the intranasal incisions to access the cartilage and bone framework, modify tip definition, reduce or refine the nasal dorsum, correct a deviated septum, or address asymmetry — without the exposure advantage of open surgery. This demands a higher level of tactile precision and spatial understanding of nasal anatomy, and is particularly suited to patients with primary cosmetic concerns who do not require the extensive framework reconstruction that sometimes necessitates the wider access of an open approach. Dr. Motykie uses computer imaging software during consultations to simulate post-operative appearance and align patient expectations with surgical planning. Approximately fifty per cent of the practice's rhinoplasty volume consists of revision cases — patients who have undergone prior nasal surgery elsewhere and seek correction of unsatisfactory results, complications, or functional issues. Revision rhinoplasty is technically more demanding due to altered anatomy, scar tissue, and potentially compromised cartilage supply, and Dr. Motykie's depth of experience in this subgroup makes the practice a referral destination for complex secondary cases. Additional rhinoplasty variants performed include ethnic rhinoplasty (preserving culturally specific nasal characteristics while achieving the desired refinement), reconstructive rhinoplasty following trauma or cancer resection, and open rhinoplasty for cases where the closed approach is not appropriate. General anesthesia is standard, and the procedure runs three to four hours. Recovery involves nasal splinting for one week, and most patients return to non-strenuous activities within ten to fourteen days. The final refined result typically becomes visible at twelve months as residual swelling fully resolves.
