Upper blepharoplasty addresses excess skin (dermatochalasis) and sometimes protruding orbital fat in the upper eyelid, which can cause hooding over the pupil and peripheral vision restriction in severe cases. The incision is placed in the natural upper eyelid crease, where the resulting scar becomes nearly invisible within the lid fold. The procedure removes a precisely marked ellipse of excess skin, the underlying orbicularis strip, and repositions or removes prolapsed medial and central fat pads to restore a naturally defined upper eyelid platform. Upper blepharoplasty alone takes 45–60 minutes under local anesthesia with sedation. Lower blepharoplasty addresses under-eye bags (fat pad prolapse), dark circles related to fat herniation, and excess lower lid skin in mature patients. The transconjunctival approach (incision inside the lower lid, leaving no external scar) is used when there is no significant skin excess to remove — ideal for younger patients with only fat prolapse. The subciliary (lash line) approach is used when there is also lower lid skin redundancy requiring excision, combining fat repositioning or removal with skin tightening. Bilateral four-eyelid blepharoplasty combining upper and lower procedures runs two to three hours.
