This panel assesses thyroid function comprehensively. TSH (thyroid-stimulating hormone) is the primary screening marker — elevated TSH indicates hypothyroidism, suppressed TSH indicates hyperthyroidism. Free T3 and free T4 measure the biologically active thyroid hormones circulating in blood, unbound to proteins. The inclusion of both anti-TPO and anti-TG antibodies distinguishes this from basic thyroid panels that test only TSH or TSH plus free T4. Anti-TPO antibodies are elevated in 90% of Hashimoto thyroiditis cases — the most common cause of hypothyroidism in iodine-sufficient populations. Anti-TG antibodies provide additional diagnostic sensitivity, particularly in thyroid cancer surveillance and in the subset of Hashimoto patients who are anti-TPO negative. Early detection of thyroid autoimmunity is clinically valuable because elevated antibodies often precede overt thyroid dysfunction by years. A patient with normal TSH but elevated anti-TPO has a significantly increased risk of developing hypothyroidism over the following decade — warranting periodic monitoring. The unbound, active form of the main thyroid hormone T4. Free T4 directly reflects thyroid gland output and is used alongside TSH to diagnose and monitor thyroid disorders.

An inactive form of T3 produced when the body converts T4. Elevated reverse T3 can occur during illness, stress, or caloric restriction and may indicate impaired thyroid hormone activation despite normal TSH levels.
Antibodies against thyroid peroxidase, an enzyme essential for thyroid hormone production. Positive TPO antibodies indicate autoimmune thyroid disease, most commonly Hashimoto's thyroiditis, even before symptoms or TSH changes appear.
