This panel addresses the key health concerns specific to men: hormonal decline, prostate health, and cardiovascular risk. Testosterone is the primary male sex hormone, and age-related decline (1–2% per year after age 30) drives symptoms including fatigue, reduced libido, muscle loss, increased body fat, and mood changes. SHBG (sex hormone-binding globulin) binds testosterone in circulation — high SHBG reduces bioavailable testosterone even when total testosterone appears normal. DHEA-S is an adrenal androgen precursor that declines with age and is associated with immune function and general vitality. Estradiol in men is produced by aromatisation of testosterone — elevated estradiol relative to testosterone suggests increased aromatase activity, which can cause gynaecomastia and contribute to fatigue. The prostate component includes both total PSA and free PSA with the free-to-total ratio. The ratio improves diagnostic specificity — a low free-to-total PSA ratio (below 15–25%) increases the likelihood that an elevated total PSA reflects malignancy rather than benign prostatic hyperplasia. Annual PSA screening is recommended from age 50 (age 45 with family history). Thyroid function (TSH) screens for the thyroid dysfunction that often presents as fatigue and weight changes in men. The metabolic component includes a full lipidogram and inflammatory markers for cardiovascular risk stratification. A CBC provides baseline haematological health and can detect polycythaemia — a common side effect of testosterone replacement therapy.
