The liver function panel evaluates hepatocellular damage, cholestatic dysfunction, and synthetic capacity. ALT (alanine aminotransferase) is the most liver-specific enzyme — elevated ALT indicates hepatocyte damage from fatty liver disease, viral hepatitis, medications, or alcohol. AST (aspartate aminotransferase) rises with liver damage but is also found in cardiac and skeletal muscle, making the AST:ALT ratio diagnostically useful. GGT (gamma-glutamyl transferase) is particularly sensitive to alcohol-related liver damage and biliary obstruction. An isolated GGT elevation with normal other liver enzymes is a common incidental finding in moderate alcohol consumers. Total bilirubin reflects both liver conjugation function and red blood cell turnover. Mild unconjugated hyperbilirubinaemia (Gilbert syndrome) affects 5–10% of the population and is benign but important to identify to avoid unnecessary investigation. Albumin and total protein assess hepatic synthetic function — the liver produces over 90% of circulating albumin. Low albumin suggests chronic liver disease, malnutrition, or protein-losing nephropathy. Non-alcoholic fatty liver disease (NAFLD) affects an estimated 25% of European adults and is the most common reason for elevated ALT in routine screening.
