This metabolic screen goes beyond simple glucose measurement to assess insulin resistance — the pathophysiological driver of type 2 diabetes, metabolic syndrome, and PCOS. Fasting glucose provides a snapshot of current glycaemic control, while HbA1c reflects average blood sugar over the preceding 8–12 weeks, smoothing out day-to-day variation. The HOMA-IR (Homeostatic Model Assessment of Insulin Resistance) combines fasting glucose and fasting insulin in a validated mathematical model to quantify insulin resistance. A HOMA-IR above 2.5 indicates insulin resistance; above 5.0 suggests significant metabolic dysfunction. This calculation detects pre-diabetic insulin resistance years before glucose or HbA1c become abnormal — the period when lifestyle intervention is most effective. Insulin resistance drives multiple downstream metabolic effects: dyslipidaemia (elevated triglycerides, low HDL), central adiposity, elevated blood pressure, and chronic low-grade inflammation. Identifying and addressing insulin resistance early can prevent or delay progression to type 2 diabetes by 58% according to landmark prevention trials.
