This panel bridges two clinical needs: acute cardiac damage detection and long-term cardiovascular risk profiling. High-sensitivity troponin T (TnT HS) is the gold-standard biomarker for myocardial injury — it detects heart muscle damage with sensitivity sufficient to identify subclinical myocardial stress. CK-MB provides additional context for acute coronary events, though its utility has diminished with the advent of high-sensitivity troponin assays. NT-proBNP (N-terminal pro-B-type natriuretic peptide) is the primary biomarker for heart failure screening and monitoring. Elevated NT-proBNP indicates ventricular wall stress from volume or pressure overload — it has high negative predictive value, meaning a normal result effectively rules out heart failure. The chronic risk component includes a full lipidogram (total cholesterol, HDL, LDL, triglycerides) for atherosclerotic risk, CRP for systemic inflammation, and homocysteine — an independent cardiovascular risk factor associated with B-vitamin metabolism. Elevated homocysteine increases risk of coronary artery disease, stroke, and venous thromboembolism. CK (creatine kinase) total screens for muscle damage broadly, including cardiac, skeletal, and smooth muscle. It contextualises the CK-MB result and identifies non-cardiac causes of chest pain. This panel is suitable for both asymptomatic screening in high-risk individuals and as a follow-up after cardiac events.
