Multiparametric prostate MRI (mpMRI) has become the standard-of-care investigation for men with elevated PSA levels or clinical suspicion of prostate cancer. The examination at Medicover Hospital uses the 3T Magnetom 3 Neo, which provides the field strength recommended by the European Society of Urogenital Radiology for prostate imaging — 3T delivers superior spatial and contrast resolution compared to 1.5T systems, improving the detection of clinically significant cancers (Gleason grade group 2 or higher). The multiparametric protocol combines three complementary sequences: T2-weighted imaging for anatomical detail and zonal architecture; diffusion-weighted imaging (DWI) with ADC mapping to identify areas of restricted water movement characteristic of dense tumour cellularity; and dynamic contrast-enhanced (DCE) imaging to evaluate tumour vascularity. Findings are reported using the PI-RADS v2.1 scoring system, which standardises the probability of clinically significant cancer on a 1-5 scale.
