Injectable lipolysis at MEDICOM Clinic Brno uses compounds based on phosphatidylcholine (PC) and deoxycholic acid (DCA) — or DCA alone in the case of the US-approved Kybella formulation — to chemically destroy fat cell membranes in a targeted area, triggering cell lysis and subsequent clearance of the released lipids through the lymphatic system. The result is a permanent reduction of adipocytes in the treated zone, as mature fat cells do not regenerate once destroyed. Phosphatidylcholine is a natural component of cell membranes and bile, where it acts as a lipid emulsifier. When injected in concentrated form into subcutaneous fat, it disrupts adipocyte membrane integrity, causing cell contents (including stored triglycerides) to be released into the interstitial space. Deoxycholic acid, a bile salt, acts synergistically — lysing the adipocyte membrane and facilitating absorption of the released fatty acids through the lymphatics to the liver for metabolic processing. The most common treatment areas are the submental zone (double chin), the jowls, the medial knee, the inner arm, the lower abdomen (particularly the suprapubic roll), and the inner thigh. Each area is treated with a grid of intradermal or subcutaneous injections spaced approximately 1 cm apart across the target fat deposit. The volume injected is proportional to the treatment area size. Post-injection, a predictable inflammatory response produces swelling, firmness, and tenderness in the treated zone for 5–14 days as the fat cell destruction and clearance process occurs. This inflammatory phase is a sign of active lipolysis rather than a complication. Fat reduction becomes visible progressively over 4–8 weeks as the lymphatic system clears the cellular debris. Typically two to four sessions per area are required for satisfactory results, scheduled four to eight weeks apart to allow full resolution of swelling before assessing response.
