A detailed spermiogram goes beyond standard WHO parameters to assess early fertility risk indicators. Male fertility declines from the late 20s, and is acutely threatened by chemotherapy, radiotherapy, high-dose immunosuppression, certain infections, and occupational or environmental exposures. MCP positions sperm cryopreservation not as a crisis measure but as a prudent preventive step — analogous to peripheral blood stem cell banking. **Spermiogram analysis:** MCP performs a detailed spermiogram analysis (sperm analysis report) covering: - Sperm concentration (million/mL) - Total motility (progressive + non-progressive + immotile fractions) - Normal morphology (Kruger strict criteria) - Volume, viscosity, pH, liquefaction time - Optional extended panel: DNA fragmentation index (DFI), reactive oxygen species (ROS), antisperm antibody titres, and leukocyte count The extended panel is recommended for patients with borderline parameters or those with known risk factors (prior infection, varicocele, environmental exposures). **Cryopreservation:** Following analysis, the sample is cryopreserved using a controlled-rate freeze protocol and stored in liquid nitrogen at -196°C. Multiple straws are stored to ensure adequate material for multiple IVF/ICSI cycles if needed. **Genetic integration:** MUDr. Silvie Růžičková can add a reproductive genetic assessment alongside the spermiogram — including Y-chromosome microdeletion analysis (AZF regions, relevant for severely impaired spermatogenesis) and karyotype analysis.
