From the mid-40s onward, facial ageing becomes structural: the deeper supporting layer of the face (the SMAS) and its ligaments stretch, letting cheek fat descend into jowls, blunting the jawline, and deepening the nasolabial folds. Injectables can disguise early changes, but only surgical repositioning corrects established descent. **The procedure:** Through incisions concealed along the hairline and around the ear, the surgeon lifts the skin, tightens and repositions the deeper SMAS layer — the step that produces a durable, natural result — redistributes descended tissue upward, and removes only the genuinely excess skin without tension. Working on the deep layer rather than pulling skin is what separates a modern facelift from the wind-tunnel look.
