Stretch marks form when the dermis undergoes rapid mechanical stretching that outpaces the skin's natural elasticity. The dermal collagen and elastin fibres tear, and the body lays down disorganised replacement collagen in the damaged zone. In the early stages, the newly injured tissue is vascularised and the marks appear pink, red, or purple — this is the striae rubrae phase. Over time the vascularity reduces, the marks flatten and widen, and they fade to a silvery-white — striae albae. The two phases respond differently to laser treatment. For striae rubrae (red/active marks), pulsed-dye or Nd:YAG vascular laser wavelengths can selectively target the haemoglobin in the feeding blood vessels, closing them and reducing the active inflammatory phase. This diminishes colour and may partially interrupt the disorganised collagen deposition at an early enough stage to improve the ultimate cosmetic outcome. For striae albae (white/mature marks), the challenge is the opposite: the tissue is now hypopigmented and hypovascular. Fractional laser delivery stimulates fibroblasts in the thin, depleted dermis to produce new collagen and elastin, gradually restoring some thickness and surface regularity to the mark. The epidermis may also resurface with slightly more normal pigmentation over a series of sessions.
