The core male hormone panel measures total testosterone, free testosterone, sex hormone-binding globulin (SHBG), luteinising hormone (LH), follicle-stimulating hormone (FSH), and prolactin. These markers establish whether testicular function and the pituitary–gonadal axis are operating within optimal ranges. Low total or free testosterone, combined with elevated LH and FSH, typically indicates primary hypogonadism (testicular failure), while low testosterone with inappropriately low or normal gonadotrophins suggests secondary hypogonadism originating at the pituitary or hypothalamic level. Oestradiol (E2) is measured alongside testosterone because an elevated oestrogen-to-testosterone ratio is a common finding in older or overweight men and contributes to fatigue, reduced libido, gynaecomastia, and mood changes. Prolactin is assessed to exclude a pituitary adenoma as a secondary cause of low testosterone. Thyroid function (TSH, fT3, fT4) is included given the bidirectional relationship between thyroid status and testosterone levels.
