When injected directly into the submental fat pad (the localized fat deposit that produces the appearance of a double chin), deoxycholic acid disrupts the structural integrity of adipocyte cell membranes through a detergent-like mechanism, causing cell lysis and releasing the intracellular lipid content. The cellular debris and released lipids are then cleared through the local lymphatic drainage and macrophage phagocytosis over the subsequent four to eight weeks. Because deoxycholic acid destroys the adipocyte cells themselves rather than simply reducing their lipid content, the treated fat cells do not return once cleared — the results of a completed Kybella course are associated with being permanent, as destroyed fat cells cannot regenerate. The treatment requires precise injection placement in a grid pattern within the submental fat pad, staying above the mandibular border and below the dermal layer to avoid the marginal mandibular nerve branch and the overlying skin. Session duration is approximately twenty to thirty minutes. The number of vials required per session and the number of sessions depends on the volume of submental fat present.
