Hyperhidrosis — primary focal hyperhidrosis — is a recognised medical condition characterised by sweating that exceeds the body's thermoregulatory needs. Affecting approximately 2–3% of the population, it occurs most commonly in the axillae (underarms), palmar surfaces (palms) and plantar surfaces (soles), and is driven by overactivation of eccrine sweat glands by the sympathetic nervous system. For many sufferers, the condition is significantly disabling: it affects choice of clothing, limits social interaction and causes considerable psychological distress. At Klinika Kasiopea, hyperhidrosis is treated with Dysport — abobotulinumtoxinA, a botulinum toxin type A preparation with a slightly wider diffusion radius than Botox, which makes it particularly efficient for covering the distributed gland field of the axilla or palm. Dysport blocks the release of acetylcholine at the sympathetic nerve-gland junction, effectively silencing the sweat gland's ability to secrete. Crucially, only the eccrine glands producing excessive sweat are blocked — the body's overall thermoregulatory capacity is preserved because sweat glands elsewhere on the body remain fully functional. The injection procedure at the clinic is brief and well tolerated. For axillary treatment, the skin of the underarm is cleansed and the treatment field is mapped using a starch-iodine test (minor test) to identify the most active sweating zones, then Dysport is delivered in a grid pattern of 10–20 injections per axilla using fine-gauge needles. The procedure takes 15–25 minutes for both axillae. Palm and sole treatment follows the same grid approach with a denser injection pattern; topical anaesthetic cream is applied before palmar or plantar injections due to higher sensitivity in these areas. Results begin within two to five days as the toxin reaches peak effect at the nerve-gland junction, with full dryness typically established at one week. The duration of effect ranges from eight to twelve months — longer than the three to six months seen for facial botulinum toxin because sweat glands have a lower metabolic turnover than facial muscles. At twelve months, sweating gradually returns and the treatment can be repeated. Many patients find they need slightly less product with each successive treatment as the glands adapt. Aftercare is minimal: patients avoid saunas, sunbathing, tanning beds and vigorous exercise for one week to prevent premature diffusion of the toxin into adjacent tissue. No dressings are required and normal activities can be resumed immediately after the procedure. There is no hospitalization, no scarring and no recovery period. The treatment is available for adults only. Contraindications include pregnancy and breastfeeding, active inflammation in the treatment area, acute systemic illness and neuromuscular diseases.
