Two structural factors most commonly cause ear prominence: an underdeveloped antihelical fold (the inner cartilage ridge that normally curves the upper ear), and/or an overly deep conchal bowl (the cup of the ear). Both factors may coexist. The surgical approach involves a small incision hidden behind the ear, through which the cartilage is accessed. For antihelical fold correction, cartilage-sparing sutures (Mustardé technique) fold and secure the cartilage into its natural position. For conchal reduction, excess cartilage is excised or scored. Both techniques are frequently combined. Operating time is approximately 1–1.5 hours under general or local anaesthesia depending on age and patient preference. A head bandage is worn for 7–10 days post-operatively to protect the repositioned cartilage while initial healing occurs. A protective headband is recommended at night for 6–8 weeks. Minor discomfort and numbness are typical for several weeks; permanent numbness is rare. Scars behind the ear are virtually invisible once healed.
