Candidates are required to have stable breast size for at least 6 months before surgery. The surgical technique depends on the composition of the enlarged tissue. Pure glandular gynecomastia (firm, disc-shaped tissue directly behind the nipple) requires surgical excision via a periareolar incision — the scar is placed at the lower half of the areola border and becomes nearly imperceptible once healed. Fatty gynecomastia responds to liposuction alone; mixed gynecomastia (both glandular tissue and excess fat) benefits from a combined approach: glandular excision plus vibration liposuction of the chest (55,000 Kč). The procedure is performed under general anaesthesia in ISCARE's day-surgery suite and takes approximately 1–2 hours depending on the combination used. Patients typically stay for monitoring for several hours post-operatively and are discharged the same day with a compression vest to be worn for 4–6 weeks.
