This panel combines the breadth of the Pakiet II comprehensive screen with thyroid-stimulating hormone (TSH) — a clinically important addition given that thyroid dysfunction is 5-8 times more prevalent in women than men. Hashimoto thyroiditis, the most common cause of hypothyroidism in Europe, has a female-to-male ratio of approximately 10:1, and subclinical hypothyroidism affects 5-10% of women over 40. TSH is the single most sensitive screening marker for thyroid dysfunction. Elevated TSH (above 4.0-4.5 mIU/L depending on laboratory reference range) indicates primary hypothyroidism, while suppressed TSH (below 0.4 mIU/L) suggests hyperthyroidism. Thyroid disease commonly presents with non-specific symptoms — fatigue, weight changes, menstrual irregularity, mood disturbance, hair loss — that overlap with many other conditions, making biochemical screening essential for accurate diagnosis. The Pakiet II base provides haematological assessment (CBC with ESR for inflammation screening), metabolic markers (fasting glucose for diabetes screening, electrolytes and magnesium for cellular function), hepatic function (AST, ALT, amylase), renal function (creatinine with eGFR), cardiovascular risk (full lipidogram), and iron status. Iron deficiency deserves particular attention in women of reproductive age. Menstrual blood loss of 80 mL or more per cycle — the threshold defining menorrhagia — depletes iron stores rapidly. The CBC component detects overt anaemia, while the iron measurement identifies depletion before haemoglobin drops.
