This three-marker thyroid panel provides a complete functional assessment of the hypothalamic-pituitary-thyroid axis. TSH (thyroid-stimulating hormone) is secreted by the anterior pituitary and is the most sensitive single marker for thyroid dysfunction. Elevated TSH indicates primary hypothyroidism — the thyroid gland is underperforming and the pituitary is compensating by producing more stimulation. Suppressed TSH indicates hyperthyroidism — excess thyroid hormone feeds back to shut down pituitary TSH production. Free T3 (triiodothyronine) and free T4 (thyroxine) measure the biologically active, unbound fractions of circulating thyroid hormones. T4 is the predominant secretory product of the thyroid gland and serves as a prohormone, converted to the more metabolically active T3 in peripheral tissues. Measuring both free hormones distinguishes between different patterns of thyroid dysfunction — for example, isolated T3 thyrotoxicosis (elevated T3 with normal T4) is common in early Graves disease and toxic nodular goitre. Subclinical thyroid disease (abnormal TSH with normal free hormones) is found in 4-10% of the general population and is more prevalent in women and older adults. Symptoms are often subtle — fatigue, weight changes, cold intolerance, dry skin, constipation for hypothyroidism; anxiety, tremor, heat intolerance, palpitations for hyperthyroidism. Free T3 (Triiodothyronine) pg/mL

The active form of thyroid hormone responsible for regulating metabolism, energy, and body temperature. Free T3 is converted from T4 in tissues and is three to four times more potent than T4.

The unbound, active form of the main thyroid hormone T4. Free T4 directly reflects thyroid gland output and is used alongside TSH to diagnose and monitor thyroid disorders.
