This panel extends the Pakiet II comprehensive metabolic screen with prostate-specific antigen (PSA) — the primary biomarker for prostate cancer screening. Prostate cancer is the most commonly diagnosed non-skin malignancy in European men and the second leading cause of cancer death. PSA screening enables detection at curable, organ-confined stages before symptoms develop. Total PSA above 4.0 ng/mL has traditionally been used as the referral threshold for urological evaluation. However, age-specific reference ranges improve clinical utility — expected PSA increases with age due to benign prostatic hyperplasia (BPH), and a value of 2.5 ng/mL in a man under 50 may warrant investigation. The European Association of Urology recommends baseline PSA measurement at age 40-45, with screening intervals individualised based on the result. Importantly, PSA is organ-specific but not cancer-specific. Elevated PSA can result from BPH, prostatitis, recent ejaculation, cycling, or urinary tract infection. This is why elevated screening PSA triggers further investigation (MRI, biopsy) rather than treatment directly. The Pakiet II base provides broad metabolic and organ function coverage: haematology (CBC with ESR), diabetes screening (fasting glucose), cardiovascular risk assessment (full lipidogram), hepatic function (AST, ALT, amylase), renal function (creatinine with eGFR), electrolyte balance (Na, K, Cl, Mg), and iron status. Cardiovascular disease is the leading cause of death in European men, and the lipidogram component enables SCORE2 risk calculation when combined with blood pressure and smoking history. The metabolic markers screen for type 2 diabetes and metabolic syndrome — both substantially more prevalent in men than women.
