The 4th-generation HIV combination test represents the current standard of care for HIV screening. By simultaneously detecting HIV 1/2 antibodies and the p24 capsid antigen, it identifies infection earlier than antibody-only assays. The p24 antigen appears in blood approximately 2-3 weeks after infection, during the acute viraemic phase, before the adaptive immune system generates detectable antibodies at 3-6 weeks. This dual approach narrows the diagnostic window period to approximately 4 weeks for most individuals, compared to 6-12 weeks for 3rd-generation antibody-only tests. A negative result at 6 weeks post-exposure is considered conclusive by European Centre for Disease Prevention and Control guidelines. The test is performed on a standard venous blood sample and requires no fasting or special preparation. A reactive screening result is always followed by confirmatory testing — typically a Western blot or immunoblot — before a definitive diagnosis is made. Poland has a relatively low HIV prevalence (estimated 0.1% of the adult population), but late diagnosis remains a significant public health challenge — approximately 50% of new HIV diagnoses in Poland are made at an advanced stage of infection.
