This panel builds on the basic thyroid trio (TSH, fT3, fT4) with two critical autoimmune markers. Anti-TPO (anti-thyroid peroxidase) antibodies target the enzyme responsible for thyroid hormone synthesis and are elevated in approximately 90% of Hashimoto thyroiditis cases — the most common cause of hypothyroidism in iodine-replete populations. Anti-TG (anti-thyroglobulin) antibodies target the protein precursor of thyroid hormones and provide additional diagnostic sensitivity, particularly for the subset of Hashimoto patients who test anti-TPO negative. The clinical value of autoimmune screening lies in early detection. Elevated thyroid antibodies frequently precede overt thyroid dysfunction by five to ten years. A patient with normal TSH but high anti-TPO has a substantially increased annual risk of progressing to hypothyroidism — roughly 2-4% per year. This knowledge enables proactive monitoring rather than reactive treatment after symptoms have already developed. Hashimoto thyroiditis disproportionately affects women, with a female-to-male ratio of approximately 10:1. It is the most common autoimmune disease worldwide. Risk factors include family history, other autoimmune conditions (type 1 diabetes, coeliac disease, vitiligo), and pregnancy — postpartum thyroiditis affects 5-10% of women. For patients with suspected Graves disease, anti-TSH receptor antibodies (TRAb) provide more specific confirmation but are not included in this standard panel. InterLab can order TRAb as an individual add-on if clinically indicated. The 170 PLN price point makes comprehensive autoimmune thyroid screening accessible for routine use rather than reserving it for patients who already have overt dysfunction.
