Pakiet II significantly expands on the basic screen by adding metabolic, renal, and nutritional markers. The CBC is upgraded to include OB (ESR — erythrocyte sedimentation rate), a non-specific inflammation marker that remains clinically useful for screening polymyalgia rheumatica, temporal arteritis, and chronic infections. Fasting glucose screens for diabetes mellitus (diagnostic threshold 7.0 mmol/L or 126 mg/dL) and impaired fasting glucose (5.6-6.9 mmol/L), which identifies pre-diabetic states amenable to lifestyle intervention. Electrolytes (sodium, potassium, chloride) assess fluid balance and renal tubular function. Magnesium deficiency is underdiagnosed and causes muscle cramps, fatigue, cardiac arrhythmias, and poor sleep — particularly common in patients on proton pump inhibitors or diuretics. Creatinine with estimated glomerular filtration rate (eGFR) is the primary screening tool for chronic kidney disease. An eGFR below 60 mL/min sustained for three or more months defines CKD stage 3, which affects an estimated 10% of adults over 65. Amylase screens for pancreatic pathology — elevated levels indicate pancreatitis or pancreatic duct obstruction. Iron status assesses the most common nutritional deficiency worldwide.
